New Learner Enrollment Packet

Completing the New Learner Enrollment Packet is the first step in your BCBA creating a personalized therapy plan for your child. Because we want to get the most accurate picture of your learner's needs, the forms are comprehensive overview to help us prepare for the Initial Assessment.

Most families find that completing the full enrollment packet takes approximately 1 to 2 hours. To make the process smoother, we recommend having the following documents readily available:

  • Court Documents (if applicable).

  • Diagnostic Assessment. A diagnosis of Autism Spectrum Disorder (ASD) that indicates an assessment tool such as CARS-2, ADOS-2, or ADI-R.

  • Copy of ALL insurance cards. Before beginning services, verification of benefits is required. A photocopy of the front and back of the insurance card(s) is sufficient. A record of every insurance card is required unless you are fully paying out of pocket.

  • Copy of Individualized Family Service Plan (IFSP) or Individualized Education Plan (IEP). If your child is currently enrolled in school, a copy of their IEP is required to submit as part of the authorization process. If your child is not in school, but enrolled in Birth-to-3 services, the IFSP is required.

  • Release of Information (ROI) for other service providers. If your child is currently enrolled, or had previously been enrolled, in other services like Speech Therapy, Occupational Therapy, or Physical Therapy, we request a ROI be completed for each provider to help us gain a complete therapeutic history, and help us collaborate with other providers. However, this step is optional. If your insurance plan requires a prescription for ABA services, we will need an ROI signed with your primary care physician.
